Make a Referral

To make a patient referral for aquatic therapy and exercise programs, please download the referral form. Please return via fax or email!

All referrals for Warm Water Physical Therapy must be submitted with authorization from a physician. Patients may be referred for:

Aquatic Physical Therapy One on one treatment, twice a week with a licensed physical therapist.

Community Swim Unstructured, participant benefits from warm water.
